This survey will take around 3-5 minutes of your time. Your information will be used for engagement purposes as per our Privacy Policy.

Thank you for participating in this survey. The information you provide will be used solely for the purpose of understanding the health and wellbeing needs of senior residents. Your responses will be deidentified and aggregated to ensure privacy. The data may be shared with NSW Health for analysis and reporting purposes, but no individual responses will be identifiable.

Data retention: Your deidentified responses will be securely stored and will only be used for research and reporting purposes related to this survey. After this time, the data will be securely deleted or anonymized, in accordance with our data retention policy.
